What does out-of-network provider mean?

Out-of-Network means that I do not participate with insurance plan. However, many insurance companies provide reimbursement for out-of-network providers. Check with your insurance company, prior to making an appointment, to determine if they reimburse for services. Below are helpful questions to ask when you call your insurance provider.

Questions to Ask Your Insurer to Determine Reimbursement Eligibility:

Do I have out-of-network benefits to see a licensed therapist? If yes, what percentage do you cover?

How many sessions are covered per year?

Do I have a deductible and has it been met?

What CPT codes do you reimburse for? My therapist uses a CPT code for a 60 minute session (90837).

What forms do I need to submit to qualify for reimbursement?
